Please complete the documents fully (everything filled out and no blank spaces) and fax back to 1 (888)-277-1820 for our underwriters to review. A clear copy of a state issued ID must be sent with all faxed forms. The original forms and the copy of the State ID must be mailed to our office and received no later than 5 days from date the bond was posted to avoid re-arrest.
